What to bring to have your taxes prepared
1) Names exactly as shown on social security card, social security numbers and dates of birth of
everyone listed on your tax return.
2) Information about your stimulus payment
3) All W-2's 1098's 1099's 1099G's, Unemployment Statement, Social Security Statement, Interest Income, and any other income including non taxable income that you did not receive a tax document.
4) 1099-SA for any HSA Distributions and any contributions made to an HSA not through payroll.
5) Form 1095-A If you purchased health insurance through the health insurance market place.
4) Form 1095 - an information form provided to you regarding you health insurance coverage.
6) End of year brokerage statements that show any sales or exchanges of investments including
gains and losses on those investments.
7) Schedule K-1's from Partnerships, estates or S-Corporations.
Rental Income and expenses, cost of assets and date placed in service - for depreciation.
8) W2G gambling winnings and a total of all losses.
9) Alimony amounts received/paid (if paid we need ex-spouses social security number)
10) Teacher out of pocket classroom expenses.
11) Records showing any contributions to Traditional or Roth IRA's and Health Savings Accounts.
12) If you sold capital assets such as real estate, stocks or mutual funds, we will need to know
your cost or other basis, date of purchase, inheritance or gift.
13) For child or dependent care credit we will need the amount paid for each dependent along
with the name, complete address and federal identification number of each provider.
14) Itemized Deductions-
-Medical expenses - health insurance premiums, doctors, hospitals, co-pays, eye
exams/glasses, dental exams/work, home improvements for medical reasons, and
mileage driven for medical purposes.
15) Real Estate taxes on principle and second residence.
16) Sales tax paid - if you purchased a new (or used) vehicle bring the purchase information -
purchase price and sales tax paid.
17) State and local income tax paid.
18) Vehicle Registration Fees.
19) Mortgage interest and points, mortgage insurance premiums (PMI).
20) Investment Interest.
21) Charitable Contributions - if you have receipts. Cash and non-cash separate, if $500 or more
in non-cash we need to know who it was given to, the date given, what it was and the
value.
22) Casualty theft or losses.
23) Investment management fees.
24) Amounts paid for post secondary education such as college (from 1098T), books, supplies
and equipment for you and/or your dependent.
25) If you are self-employed/independent contractor/sub-contractor we will need to know the
gross amount of income and your expenses separated into different categories.

if you qualify for a deduction for any taxes paid at closing.
